Overview
- Coordinate telephonic case management for CHHA patients, ensuring high-quality home health services in compliance with Department of Health regulations.
- Develop and implement Physician Plans of Care in collaboration with healthcare professionals for assigned cases.
- Support the agency's mission by adhering to regulatory guidelines and maintaining high standards of care.
- Work in an on-site environment during standard business hours, Monday through Friday, 9 AM to 5 PM.
- Provide telephonic support and coordination for field nurses transitioning into case management roles.
- Ensure seamless coordination of home health services for assigned patient caseloads.
- Collaborate with interdisciplinary teams to deliver comprehensive patient care solutions.
- Maintain accurate documentation and records to support compliance and quality assurance initiatives.
Key Responsibilities & Duties
- Manage telephonic case coordination for CHHA patients, ensuring timely and effective service delivery.
- Develop and oversee Physician Plans of Care, ensuring alignment with patient needs and regulatory standards.
- Monitor and evaluate the quality of home health services provided to assigned patients.
- Collaborate with healthcare professionals to address patient concerns and optimize care plans.
- Support field nurses transitioning into telephonic case management roles through training and mentorship.
- Ensure compliance with Department of Health regulations and agency standards in all case management activities.
- Maintain detailed and accurate patient records to support regulatory compliance and quality assurance.
- Participate in interdisciplinary team meetings to enhance patient care coordination and outcomes.
Job Requirements
- Associate of Applied Science (AAS) degree in Nursing or related field.
- Licensed Practical Nurse (LPN) or Registered Nurse (RN) certification required.
- Minimum of 2 years of experience in CHHA case management; 4 years preferred.
- Strong knowledge of Department of Health regulations and home health service standards.
- Experience in telephonic case management and interdisciplinary team collaboration.
- Excellent organizational and communication skills to manage patient caseloads effectively.
- Proficiency in maintaining accurate documentation and compliance records.
- Ability to work on-site during standard business hours, Monday through Friday.
