Job Description

Overview

  • Conduct comprehensive assessments using the Uniform Assessment System (UAS-NY) for members' clinical, psychological, and social needs.
  • Ensure accurate documentation and compliance with New York State Department of Health regulations.
  • Collaborate with interdisciplinary care teams to develop individualized care plans.
  • Perform hybrid assessments, including telephonic and in-home visits, accommodating patient preferences.
  • Complete enrollment and reassessment cases within mandated timeframes and departmental policies.
  • Explain program benefits and rules to members in an accessible manner.
  • Provide training and support during onboarding, ensuring readiness for field assessments.
  • Maintain communication with members, families, caregivers, and healthcare professionals regarding care plans.

Key Responsibilities & Duties

  • Conduct initial enrollment assessments and reassessments for members using UAS-NY protocols.
  • Determine eligibility and establish safe, effective plans of care for members.
  • Communicate assessment outcomes to referral sources and enrollment representatives.
  • Complete all necessary documentation, including DOH-approved forms, within regulatory timeframes.
  • Address urgent requests, safety concerns, and clinical care needs promptly.
  • Collaborate with care management teams to ensure comprehensive member support.
  • Provide clear communication regarding program rules, benefits, and responsibilities.
  • Support members in navigating healthcare services and Medicare benefits.

Job Requirements

  • Associate Degree in Nursing required; Bachelor's Degree preferred.
  • Active RN license with a minimum of one year of experience in community health or managed care.
  • Proficiency in Microsoft Word, Excel, and UAS-NY systems.
  • Bilingual skills in Bangla, Hindi, Urdu, Mandarin, Cantonese, or Spanish preferred.
  • Comfortable managing primary and secondary territories for assessments.
  • Ability to conduct hybrid assessments, including telephonic and in-home visits.
  • Strong organizational skills to manage multiple cases and meet productivity targets.
  • Experience with MLTC or homecare assessments highly desirable.
