Job Description

Overview

  • Provide direct patient care through occupational therapy treatments for adult home care patients.
  • Coordinate patient care plans with rehabilitation management and ensure compliance with regulations.
  • Submit required documentation promptly, adhering to agency policies and procedures.
  • Collaborate with interdisciplinary teams to optimize patient outcomes and care quality.
  • Evaluate patient needs and develop individualized therapy plans to promote recovery.
  • Educate patients and families on therapy techniques and home care practices.
  • Maintain accurate records of therapy sessions and patient progress.

Key Responsibilities & Duties

Job Requirements
