Key Responsibilities & Duties
- Conduct thorough patient assessments, diagnoses, and treatments across various age groups.
- Provide timely follow-up on patient needs, test results, and referrals.
- Document patient care accurately in the electronic health record system.
- Collaborate with healthcare professionals to ensure coordinated and effective care delivery.
- Participate in case conferences and contribute to quality improvement initiatives.
- Engage in continuing medical education to stay updated with best practices.
- Make appropriate referrals for specialized care, ensuring patient needs are met.
Job Requirements
- Doctoral degree (DO, MD, or NP) with New York State Certification/Licensure.
- Current NYS Registration and DEA Controlled Substances Registration Certificate.
- Board certified or eligible in Family Medicine, Internal Medicine, or Infectious Disease.
- Minimum of 2 years of primary care experience; 3 years preferred.
- Experience in HIV and transgender care is highly desirable.
- Strong collaboration skills in a team-based model of care.
- Commitment to accessible, patient-centered primary care delivery.
- Bilingual proficiency (Spanish/English) is a plus.
