Overview

- Conduct thorough medical reviews of clinical assessments to finalize wellness plans for program clients.
- Provide guidance to clients, facilitating access to essential services and reducing barriers to compliance.
- Support staff with clinical expertise to achieve program goals and enhance service delivery.
- Utilize knowledge of medical and behavioral health diagnoses to optimize client care and outcomes.
- Work on-site to ensure operational efficiency and provide direct support to clients and staff.
- Engage in continuous professional development to stay updated with healthcare advancements and best practices.
Key Responsibilities & Duties
- Perform detailed medical reviews of clinical assessments and reassessments to ensure accuracy and compliance.
- Develop and implement wellness plans tailored to individual client needs and program objectives.
- Assist clients in navigating healthcare systems and accessing necessary services to improve compliance.
- Provide clinical expertise and support to staff, fostering a collaborative and effective work environment.

- Maintain up-to-date knowledge of SSA disability application processes and related healthcare regulations.
Job Requirements
- Doctor of Medicine (MD) degree with an unrestricted license to practice in New York State.
- Board Certification by the American Board of Medical Specialties (ABMS) or American Osteopathic Association (AOA).
- Minimum of two years of professional experience in medical or clinical practice.
- Proficiency in managing medical and behavioral health diagnoses within a clinical setting.
- Familiarity with NYC HRA and SSA disability application processes is preferred.
- Experience with healthcare software systems such as SEAMS, EPIC, PSYCKES, and Bronx RHIO.
- Strong computer skills, including proficiency in Microsoft Office applications.
- Ability to motivate and assist clients in compliance processes effectively.
