Overview
- Provide comprehensive women’s health services, including preventive care, chronic disease management, and contraception, in a patient-focused environment.
- Collaborate with interdisciplinary teams to deliver culturally competent care tailored to diverse populations.
- Conduct well-woman exams and educate patients on preventive health and wellness strategies.
- Insert and manage long-acting reversible contraception devices with precision and patient comfort.
- Engage in community-based care, meeting patients in accessible locations to enhance healthcare delivery.
- Utilize evidence-based practices to manage complex medical and mental health conditions effectively.
- Promote health education and preventive care to improve patient outcomes and quality of life.
- Support patient empowerment through personalized care plans and continuous education.
Key Responsibilities & Duties
- Perform comprehensive assessments to identify patients’ healthcare needs and develop tailored care plans.
- Coordinate care transitions and facilitate communication among patients, caregivers, and healthcare teams.
- Educate patients on contraception options and provide prescriptions as necessary.
- Insert intrauterine devices and other implanted contraception devices with expertise and care.
- Monitor and manage chronic medical and mental health conditions using evidence-based guidelines.
- Collaborate with interdisciplinary teams to ensure high-quality outcomes for patients and families.
- Maintain accurate and detailed patient records to support effective care delivery.
- Advocate for patient health and well-being through education and community outreach programs.
Job Requirements
- Master of Science in Nursing with certification as WHNP or FNP.
- Minimum of 1 year of experience in women’s health; 2 years preferred.
- Proficiency in conducting well-woman exams and managing contraception effectively.
- Experience in community-based or home care settings is advantageous.
- Strong interpersonal skills to engage effectively with patients and caregivers.
- Knowledge of evidence-based practices for chronic disease management.
- Ability to work independently and adapt to flexible schedules.
