Overview

  • Provide comprehensive women’s health services, including preventive care, chronic disease management, and contraception, in a patient-focused environment.
  • Collaborate with interdisciplinary teams to deliver culturally competent care tailored to diverse populations.
  • Conduct well-woman exams and educate patients on preventive health and wellness strategies.
  • Insert and manage long-acting reversible contraception devices with precision and patient comfort.
  • Engage in community-based care, meeting patients in accessible locations to enhance healthcare delivery.
  • Utilize evidence-based practices to manage complex medical and mental health conditions effectively.
  • Promote health education and preventive care to improve patient outcomes and quality of life.
  • Support patient empowerment through personalized care plans and continuous education.

Key Responsibilities & Duties

  • Perform comprehensive assessments to identify patients’ healthcare needs and develop tailored care plans.
  • Coordinate care transitions and facilitate communication among patients, caregivers, and healthcare teams.
  • Educate patients on contraception options and provide prescriptions as necessary.
  • Insert intrauterine devices and other implanted contraception devices with expertise and care.
  • Monitor and manage chronic medical and mental health conditions using evidence-based guidelines.
  • Collaborate with interdisciplinary teams to ensure high-quality outcomes for patients and families.
  • Maintain accurate and detailed patient records to support effective care delivery.
  • Advocate for patient health and well-being through education and community outreach programs.

Job Requirements

  • Master of Science in Nursing with certification as WHNP or FNP.
  • Minimum of 1 year of experience in women’s health; 2 years preferred.
  • Proficiency in conducting well-woman exams and managing contraception effectively.
  • Experience in community-based or home care settings is advantageous.
  • Strong interpersonal skills to engage effectively with patients and caregivers.
  • Knowledge of evidence-based practices for chronic disease management.
  • Ability to work independently and adapt to flexible schedules.
