Registered Nurse Case Manager – Maternal Health

Overview

• Coordinate maternal health care for program participants, focusing on perinatal wellness and whole-person support.
• Conduct clinical assessments to identify health needs and develop personalized care plans.
• Collaborate with interdisciplinary teams to ensure comprehensive and effective care delivery.
• Facilitate transitions of care and provide wraparound support during critical periods.
• Engage with community health workers to deliver targeted health education and interventions.
• Maintain accurate and compliant documentation of care plans and participant interactions.
• Promote health equity and access for underserved populations in maternal health.
• Participate in ongoing professional development to enhance clinical expertise.

Key Responsibilities & Duties

• Conduct assessments to evaluate participant health status and care requirements.
• Develop and update individualized care plans in collaboration with participants and teams.
• Coordinate interdisciplinary teams to address medical, behavioral, and social health determinants.
• Monitor participant progress and address health deterioration or care gaps promptly.
• Facilitate effective communication among care team members for seamless service delivery.
• Provide guidance and education on maternal health and wellness practices.
• Ensure adherence to program policies and regulatory standards in all activities.
• Support community outreach initiatives to enhance program awareness and engagement.

Job Requirements

• Bachelor of Science in Nursing (BSN) with active RN licensure required.
• Minimum 1 year of experience in case management or maternal health; 2 years preferred.
• Strong knowledge of maternal health practices and perinatal care protocols.
• Proficiency in clinical assessments and care planning using evidence-based tools.
• Ability to work in field-based settings and adapt to participant needs.
• Excellent communication and collaboration skills for interdisciplinary teamwork.
• Commitment to health equity and improving access to care for underserved populations.
• Flexibility to work Monday through Friday with potential for varied field locations.
