Overview
- Coordinate maternal health care for program participants, focusing on perinatal wellness and whole-person support.
- Conduct clinical assessments to identify health needs and develop personalized care plans.

- Facilitate transitions of care and provide wraparound support during critical periods.

Key Responsibilities & Duties
- Conduct assessments to evaluate participant health status and care requirements.
- Develop and update individualized care plans in collaboration with participants and teams.
- Coordinate interdisciplinary teams to address medical, behavioral, and social health determinants.
- Monitor participant progress and address health deterioration or care gaps promptly.
- Facilitate effective communication among care team members for seamless service delivery.
- Provide guidance and education on maternal health and wellness practices.
- Ensure adherence to program policies and regulatory standards in all activities.
- Support community outreach initiatives to enhance program awareness and engagement.
Job Requirements
- Bachelor of Science in Nursing (BSN) with active RN licensure required.
- Minimum 1 year of experience in case management or maternal health; 2 years preferred.
- Strong knowledge of maternal health practices and perinatal care protocols.
- Proficiency in clinical assessments and care planning using evidence-based tools.
- Ability to work in field-based settings and adapt to participant needs.
- Excellent communication and collaboration skills for interdisciplinary teamwork.
- Commitment to health equity and improving access to care for underserved populations.
- Flexibility to work Monday through Friday with potential for varied field locations.
