Key Responsibilities & Duties
- Act as primary medical case manager, coordinating health needs and treatments for assigned clients.
- Conduct health and vitals checks, collaborating with nursing leadership and team members.
- Maintain medication administration records and controlled substance logs, ensuring compliance and adequate supply.
- Provide health education to clients, advocating for their preferences and goals.
- Participate in treatment team meetings and case conferences, sharing findings and client needs.
- Deliver emergency first aid and CPR assistance when required.
- Coordinate referrals for medical treatment and psychiatric rehabilitation as needed.
- Perform related duties as assigned to support the facility's operations.
Job Requirements
- Bachelor of Science in Nursing (BSN) and active RN license required.
- Minimum of 1 year of nursing experience, with 3 years preferred.
- Proficiency in managing medical case files and medication administration records.
- Strong collaboration skills to work effectively with multidisciplinary teams.
- Ability to provide health education and advocate for client needs.
- Knowledge of detoxification and rehabilitation processes is advantageous.
- Commitment to maintaining high standards of client care and safety.
- Availability to work onsite, including weekends as scheduled.
