Job Description

Overview

  • Conduct comprehensive assessments using the Uniform Assessment System (UAS-NY) to evaluate members' clinical, psychological, and social needs.
  • Collaborate with interdisciplinary teams to develop personalized care plans tailored to members' preferences and requirements.
  • Perform hybrid assessments, including telephonic and in-home visits, ensuring compliance with regulatory standards.
  • Educate members and their families about program benefits, eligibility criteria, and care options in an accessible manner.
  • Ensure accurate documentation and timely submission of assessment results to relevant teams.
  • Conduct initial enrollment assessments and periodic reassessments, focusing on safety, eligibility, and care planning.
  • Support members in accessing services, including CDPAS enrollment, and coordinate with healthcare professionals.
  • Participate in ongoing training sessions to stay updated on policies, procedures, and best practices.

Key Responsibilities & Duties

  • Conduct UAS-NY assessments to determine member needs and eligibility for services, ensuring compliance with guidelines.
  • Develop and communicate individualized care plans to members, families, and interdisciplinary teams.
  • Ensure timely completion and accurate documentation of all assessments within mandated timeframes.
  • Coordinate with interdisciplinary teams to address urgent care needs and safety concerns effectively.
  • Educate members about program benefits, responsibilities, and care options during assessments.
  • Conduct follow-up assessments to ensure members receive appropriate care and services.
  • Maintain compliance with Department of Health policies and regulations during all assessment activities.
  • Engage in professional development activities to enhance assessment skills and knowledge.

Job Requirements

  • Associate Degree in Nursing (AAS) required; Bachelor's Degree in Nursing preferred.
  • Minimum of two years of experience in community health, managed care, or related fields.
  • Proficiency in Microsoft Word, Excel, and UAS-NY systems for documentation and reporting.
  • Bilingual skills in Bangla, Hindi, Urdu, Mandarin, Cantonese, or Spanish are highly desirable.
  • Valid driver's license and willingness to travel within assigned territories for in-home assessments.
  • Comfortable conducting both telephonic and in-home assessments in a hybrid work environment.
  • Strong communication and interpersonal skills to interact effectively with diverse populations.
  • Ability to manage multiple cases and meet productivity targets efficiently.
