Overview
- Provide compassionate nursing care to hospice patients, ensuring dignity and comfort during end-of-life stages.
- Conduct comprehensive assessments to create personalized care plans addressing physical and emotional needs.

Key Responsibilities & Duties
- Perform initial and ongoing assessments to determine hospice patient care needs.
- Develop and implement individualized nursing care plans based on patient conditions.

- Monitor patient progress and adjust care plans as necessary to meet evolving needs.

Job Requirements
- Associate of Applied Science (AAS) in Nursing or related field required.
- Minimum of 1 year of clinical experience, preferably in hospice or acute care settings.
- Preferred 6 years of experience in hospice or related nursing roles.
- Strong ability to collaborate within interdisciplinary team environments.
- Proficient in clinical data input and maintaining accurate documentation.
- Excellent communication, teaching, and listening skills.
- Knowledge of hospice care standards and practices preferred.
