Job Description

Overview

  • Provide compassionate nursing care to hospice patients, ensuring dignity and comfort during end-of-life stages.
  • Conduct comprehensive assessments to create personalized care plans addressing physical and emotional needs.
  • Collaborate with interdisciplinary teams to deliver holistic and patient-centered care.
  • Educate patients and families on hospice care processes and support resources.
  • Supervise and mentor home health aides and personal care workers to ensure quality care delivery.
  • Maintain accurate documentation and input clinical data proficiently.
  • Adhere to hospice care standards, policies, and regulations consistently.
  • Foster a supportive environment for patients and families during challenging times.

Key Responsibilities & Duties

  • Perform initial and ongoing assessments to determine hospice patient care needs.
  • Develop and implement individualized nursing care plans based on patient conditions.
  • Coordinate with physicians and interdisciplinary teams to ensure optimal care delivery.
  • Monitor patient progress and adjust care plans as necessary to meet evolving needs.
  • Provide education and emotional support to patients and families regarding hospice care.
  • Supervise and train home health aides and personal care workers effectively.
  • Ensure timely and accurate documentation of patient care activities and clinical data.
  • Maintain compliance with hospice care standards, policies, and regulations.

Job Requirements

  • Associate of Applied Science (AAS) in Nursing or related field required.
  • Minimum of 1 year of clinical experience, preferably in hospice or acute care settings.
  • Preferred 6 years of experience in hospice or related nursing roles.
  • Strong ability to collaborate within interdisciplinary team environments.
  • Proficient in clinical data input and maintaining accurate documentation.
  • Excellent communication, teaching, and listening skills.
  • Knowledge of hospice care standards and practices preferred.
