Overview
- Deliver comprehensive case management services, addressing medical, psychosocial, and spiritual needs of members effectively.
- Develop and implement personalized care plans to ensure seamless healthcare delivery.

- Conduct assessments through telephonic and in-home visits to evaluate member conditions.
- Educate members on health management, preventive care, and lifestyle enhancements.

- Participate in care team meetings to address member conditions and coordinate care.
Key Responsibilities & Duties
- Conduct initial and ongoing assessments, utilizing UAS-NY data for informed care planning.
- Develop, revise, and monitor personalized service plans based on member needs and changes.
- Respond to sentinel events and adapt care plans to address evolving member conditions.
- Educate members on disease management, preventive measures, and quality of life improvements.
- Collaborate with healthcare teams to ensure coordinated and effective care delivery.
- Utilize available resources to address member needs and enhance care outcomes.
- Present cases and strategies during interdisciplinary care team meetings.
- Maintain compliance with Medicare and Medicaid regulations in all care processes.
Job Requirements
- Associate degree in nursing or related field; BSN preferred.
- Current RN license in New York State; CCM certification is advantageous.
- Three to five years of nursing experience in relevant healthcare settings.
- Familiarity with interdisciplinary care delivery environments and healthcare regulations.
- Proficient in conducting assessments and developing effective care plans.
- Strong communication skills for collaboration with diverse healthcare teams.
- Ability to navigate community resources and benefit structures effectively.
