Job Description

Overview

  • Deliver comprehensive case management services, addressing medical, psychosocial, and spiritual needs of members effectively.
  • Develop and implement personalized care plans to ensure seamless healthcare delivery.
  • Collaborate with interdisciplinary teams to optimize care strategies and outcomes.
  • Conduct assessments through telephonic and in-home visits to evaluate member conditions.
  • Educate members on health management, preventive care, and lifestyle enhancements.
  • Ensure compliance with healthcare regulations and standards in all service delivery.
  • Utilize community resources and benefit structures to meet diverse member needs.
  • Participate in care team meetings to address member conditions and coordinate care.

Key Responsibilities & Duties

  • Conduct initial and ongoing assessments, utilizing UAS-NY data for informed care planning.
  • Develop, revise, and monitor personalized service plans based on member needs and changes.
  • Respond to sentinel events and adapt care plans to address evolving member conditions.
  • Educate members on disease management, preventive measures, and quality of life improvements.
  • Collaborate with healthcare teams to ensure coordinated and effective care delivery.
  • Utilize available resources to address member needs and enhance care outcomes.
  • Present cases and strategies during interdisciplinary care team meetings.
  • Maintain compliance with Medicare and Medicaid regulations in all care processes.

Job Requirements

  • Associate degree in nursing or related field; BSN preferred.
  • Current RN license in New York State; CCM certification is advantageous.
  • Three to five years of nursing experience in relevant healthcare settings.
  • Familiarity with interdisciplinary care delivery environments and healthcare regulations.
  • Proficient in conducting assessments and developing effective care plans.
  • Strong communication skills for collaboration with diverse healthcare teams.
  • Ability to navigate community resources and benefit structures effectively.
