Overview

- Conduct comprehensive assessments to develop personalized care plans addressing physical, emotional, and psychosocial needs.
- Collaborate with interdisciplinary teams to deliver holistic care and optimize patient outcomes.
- Educate patients and families about hospice services, fostering understanding and support.
- Supervise and mentor healthcare staff to ensure high-quality service delivery.
- Implement physician orders and adapt care plans to evolving patient conditions.

Key Responsibilities & Duties
- Conduct detailed assessments of patients' physical, emotional, and social conditions.
- Develop and implement individualized nursing care plans tailored to patient needs.
- Supervise and evaluate the performance of home health aides and other care staff.
- Coordinate with physicians to update and modify care plans as necessary.
- Provide hands-on care and emotional support to patients and their families.
- Educate families on hospice care processes and available resources.
- Ensure compliance with regulatory standards and organizational policies.

Job Requirements
- Associate of Science (AS) in Nursing or equivalent education required.
- Active Registered Nurse (RN) license with a minimum of 1 year of clinical experience.
- Preferred experience in hospice, home care, or acute care settings.
- Exceptional communication, teaching, and interpersonal skills.
- Ability to work effectively within an interdisciplinary team environment.
- Proficiency in clinical data input and accurate documentation.
