Job Description

Overview

  • Conduct comprehensive assessments using the Uniform Assessment System for New York (UAS-NY) to evaluate member needs and eligibility.
  • Collaborate with interdisciplinary teams to develop personalized care plans addressing clinical and social requirements.
  • Ensure compliance with Department of Health (DOH) regulations and maintain accurate documentation standards.
  • Perform assessments through telephonic and in-home visits, respecting member preferences and circumstances.
  • Communicate findings effectively to stakeholders, including referral sources and care management teams.
  • Educate members and caregivers on program benefits, rules, and covered services to enhance understanding.
  • Support enrollment into programs like CDPAS, ensuring all necessary documentation is completed accurately.
  • Meet productivity standards by completing assessments efficiently while maintaining high-quality outcomes.

Key Responsibilities & Duties

  • Conduct functional, psychosocial, and environmental safety assessments for members in accordance with DOH guidelines.
  • Evaluate member eligibility for services and develop safe, effective care plans tailored to individual needs.
  • Coordinate care plans with members, families, caregivers, and healthcare professionals to ensure seamless implementation.
  • Complete DOH-approved forms and documentation during face-to-face visits with members.
  • Submit timely recommendations and documentation to the Care Management team as per established deadlines.
  • Address urgent requests, safety concerns, and clinical care needs promptly to appropriate parties.
  • Participate in ongoing training sessions and adhere to departmental workflows and policies.
  • Ensure all assessments and documentation meet high-quality standards and regulatory requirements.

Job Requirements

  • Associate Degree in Nursing required; Bachelor's Degree in Nursing preferred.
  • Active Registered Nurse (RN) license in good standing is mandatory.
  • Minimum of two years of experience in Community Health, Managed Care, or related fields is required.
  • Proficiency in Microsoft Word, Excel, and UAS-NY systems is essential.
  • Strong communication skills with the ability to explain complex information clearly.
  • Bilingual skills in Spanish, Bangla, Hindi, Urdu, Mandarin, or Cantonese are preferred.
  • Comfortable working in hybrid settings, including telephonic and in-home assessments.
  • Ability to manage productivity expectations and complete assigned assessments efficiently.
