Key Responsibilities & Duties

  • Conduct detailed patient assessments using the Uniform Assessment System (UAS) framework to inform care planning.
  • Develop personalized care plans in collaboration with healthcare teams, ensuring patient-centric approaches.
  • Maintain accurate documentation and uphold compliance with healthcare regulations and standards.

Job Requirements

  • Associate of Science (AS) in Nursing or equivalent qualification is required.
  • Minimum of 2 years of nursing experience, preferably in assessment-focused roles.
  • Proficiency in utilizing the Uniform Assessment System (UAS) framework effectively.
  • Strong interpersonal and communication skills for patient and team interactions.
