Overview
- Provide essential nursing care for individuals with intellectual disabilities and mental health needs in a collaborative environment.
- Support crisis stabilization and transitional planning to ensure seamless community reintegration.
- Collaborate with interdisciplinary teams to address medical, behavioral, and psychiatric needs effectively.
- Conduct thorough nursing assessments and deliver direct interventions tailored to individual health goals.
- Monitor medication administration and compliance with healthcare regulations and standards.
- Promote health and wellness through education and coaching for individuals and support staff.
- Ensure infection control measures are implemented and adhered to consistently.
- Provide 24/7 consultation and support to address urgent care needs as required.
Key Responsibilities & Duties
- Conduct comprehensive nursing assessments and screenings for program admissions.
- Deliver direct nursing interventions and monitor for early signs of health crises.
- Coordinate follow-up care with healthcare providers and community resources.
- Facilitate hospital discharge and community reintegration ensuring continuity of care.
- Train and mentor support staff on health-related tasks and emergency protocols.
- Participate in interdisciplinary treatment planning and case conferences.
- Serve as a clinical liaison among families, providers, and program staff.
- Develop and monitor personalized health goals for individuals under care.
Job Requirements
- Bachelor of Science in Nursing (BSN) and active New York State RN license.
- Minimum 2 years of nursing experience, preferably in IDD, behavioral health, or community care.
- Familiarity with OPWDD, OMH, and DOH regulations and healthcare standards.
- Strong clinical assessment, crisis intervention, and care coordination skills.
- Excellent communication, organizational, and documentation abilities.
- Ability to work independently and collaboratively in a team-based environment.
- Valid driver’s license preferred for travel to multiple program sites.
