Registered Nurse

Overview

• Provide essential nursing care for individuals with intellectual disabilities and mental health needs in a collaborative environment.
• Support crisis stabilization and transitional planning to ensure seamless community reintegration.
• Collaborate with interdisciplinary teams to address medical, behavioral, and psychiatric needs effectively.
• Conduct thorough nursing assessments and deliver direct interventions tailored to individual health goals.
• Monitor medication administration and compliance with healthcare regulations and standards.
• Promote health and wellness through education and coaching for individuals and support staff.
• Ensure infection control measures are implemented and adhered to consistently.
• Provide 24/7 consultation and support to address urgent care needs as required.

Key Responsibilities & Duties

• Conduct comprehensive nursing assessments and screenings for program admissions.
• Deliver direct nursing interventions and monitor for early signs of health crises.
• Coordinate follow-up care with healthcare providers and community resources.
• Facilitate hospital discharge and community reintegration ensuring continuity of care.
• Train and mentor support staff on health-related tasks and emergency protocols.
• Participate in interdisciplinary treatment planning and case conferences.
• Serve as a clinical liaison among families, providers, and program staff.
• Develop and monitor personalized health goals for individuals under care.

Job Requirements

• Bachelor of Science in Nursing (BSN) and active New York State RN license.
• Minimum 2 years of nursing experience, preferably in IDD, behavioral health, or community care.
• Familiarity with OPWDD, OMH, and DOH regulations and healthcare standards.
• Strong clinical assessment, crisis intervention, and care coordination skills.
• Excellent communication, organizational, and documentation abilities.
• Ability to work independently and collaboratively in a team-based environment.
• Valid driver’s license preferred for travel to multiple program sites.
