Job Description

Overview

  • Collaborate with Physical Therapists to deliver exceptional patient care in a home care setting, enhancing mobility and overall health.
  • Assist in implementing personalized treatment plans to support patient recovery and improve their quality of life.
  • Provide hands-on therapy, including stretching, massages, and guided exercises, tailored to individual patient needs.
  • Document patient progress meticulously and communicate findings to the healthcare team for coordinated care.
  • Support patients in achieving therapy goals focusing on gait, balance, and range of motion improvements.
  • Conduct thorough patient assessments and report conditions to supervising Physical Therapists for informed decision-making.
  • Work collaboratively within a multidisciplinary team to ensure optimal patient outcomes and satisfaction.
  • Adhere to healthcare regulations and standards, maintaining compliance and promoting safe practices.

Key Responsibilities & Duties

  • Assist Physical Therapists in creating and implementing effective patient treatment plans tailored to individual needs.
  • Perform hands-on therapy techniques, including massages, stretching, and exercise assistance, to support patient recovery.
  • Guide patients through prescribed exercise programs, ensuring proper technique and encouraging progress.
  • Accurately document patient progress and communicate updates to the healthcare team for seamless care coordination.
  • Conduct patient assessments, identifying areas for improvement and reporting findings to Physical Therapists.
  • Collaborate with healthcare professionals to deliver comprehensive and compassionate patient care.
  • Maintain detailed records and ensure adherence to healthcare standards and protocols.

Job Requirements

  • Associate of Applied Science (AAS) degree in Physical Therapy Assistance or a related field.
  • Valid Physical Therapist Assistant (PTA) license is mandatory for this role.
  • Minimum of 1 year of experience required, with 2 years preferred in a similar healthcare setting.
  • Proficiency in hands-on therapy techniques and guiding patient exercise programs effectively.
  • Strong documentation and communication skills to report patient progress accurately.
  • Ability to work collaboratively within a multidisciplinary healthcare team.
