Overview

  • Manage clinical operations for a home healthcare agency, ensuring compliance, quality care, and effective case management.
  • Coordinate patient care needs, develop individualized treatment plans, and oversee field staff assignments.
  • Collaborate with healthcare professionals to establish appropriate care levels and ensure timely documentation.
  • Maintain knowledge of reimbursement practices and regulations to optimize patient services.
  • Participate in quality improvement activities and staff orientation processes.
  • Provide leadership and guidance to ensure continuity of care and regulatory compliance.
  • Engage in on-call supervisor rotation and address patient care needs promptly.
  • Utilize community resources to meet patient care requirements effectively.

Key Responsibilities & Duties

  • Coordinate and assess home healthcare needs of referred patients, ensuring timely intake processes.
  • Develop comprehensive treatment plans in collaboration with physicians and healthcare teams.
  • Ensure proper documentation and authorization for medical supplies and services.
  • Conduct case conferences to ensure effective case management and discharge planning.
  • Review admission and recertification documentation to meet patient needs.
  • Delegate tasks to support staff, providing clear and timely direction.
  • Maintain patient confidentiality and demonstrate cultural sensitivity in all interactions.
  • Participate in performance improvement activities and staff orientation programs.

Job Requirements

  • License to practice as a registered nurse; BSN preferred.
  • Minimum three years of nursing experience; two years in home health nursing preferred.
  • Knowledge of OASIS and clinical documentation standards.
  • Effective verbal and written communication skills.
  • Strong problem-solving abilities and organizational skills.
  • Commitment to quality service and customer relations.
  • Ability to manage multiple responsibilities and set priorities effectively.
  • Demonstrates working knowledge of state and federal regulations.
