Job Description

Overview

  • Provide personalized physical therapy services to patients in their homes, enhancing recovery and independence through tailored treatment plans.
  • Collaborate with multidisciplinary healthcare teams to ensure seamless care coordination and optimal patient outcomes.
  • Conduct comprehensive patient evaluations to develop individualized therapy plans addressing specific rehabilitation needs.
  • Document patient progress and therapy outcomes meticulously, maintaining compliance with healthcare standards and regulations.
  • Educate patients and caregivers on effective home therapy techniques to support ongoing recovery and independence.
  • Supervise Physical Therapy Assistants, ensuring adherence to treatment protocols and high-quality care delivery.
  • Participate in discharge planning, providing recommendations for continued care and addressing patient-specific challenges.

Key Responsibilities & Duties

  • Conduct detailed evaluations to assess patients' physical therapy needs and establish personalized treatment plans.
  • Implement therapy interventions aimed at achieving measurable short- and long-term rehabilitation goals.
  • Maintain accurate documentation of therapy sessions, progress notes, and reassessments to support care continuity.
  • Coordinate with physicians and healthcare teams to align therapy plans with overall patient care strategies.
  • Provide education to patients and caregivers on therapy techniques and home programs.
  • Supervise Physical Therapy Assistants, ensuring adherence to treatment protocols and providing mentorship as needed.
  • Respond promptly to communications and manage schedules effectively using organizational systems.
  • Participate in discharge planning, documenting patient progress and recommendations for continued care or services.

Job Requirements

  • Doctoral Degree in Physical Therapy or equivalent educational qualification.
  • Minimum of one year of professional experience in physical therapy; three years preferred.
  • Active Physical Therapist License and AHA Basic Health Care Life Support HCP Certification.
  • Proficiency in using PC and healthcare software applications for documentation and communication.
  • Strong documentation skills, ensuring timely and accurate submission of required records.
  • Physical capability to lift, push, and pull a minimum of 50 lbs., and stand for extended periods.
  • Excellent communication skills for effective patient education and team collaboration.
  • Commitment to maintaining organizational standards and delivering exceptional patient care.
