Patient Care Liaison

Overview

• Serve as a Patient Care Liaison, ensuring smooth transitions for patients into care settings while collaborating with healthcare professionals.
• Coordinate patient admissions, working closely with physicians, social workers, and case managers to ensure optimal care delivery.
• Provide compassionate support to patients and families during transitions, addressing concerns and enhancing satisfaction.
• Engage in outreach activities to promote services to hospitals and community organizations, fostering professional relationships.
• Contribute to marketing initiatives, including social media campaigns, to enhance visibility and community engagement.
• Participate in organizing educational programs and community events to strengthen organizational presence.
• Ensure adherence to healthcare policies and procedures, maintaining compliance and high standards of care.

Key Responsibilities & Duties

• Conduct daily hospital visits to establish and maintain relationships with healthcare professionals, including social workers and case managers.
• Act as a liaison between healthcare providers and agency staff, ensuring effective communication and coordination.
• Develop and implement strategies to promote services and enhance community outreach.
• Organize and deliver presentations to community groups and healthcare organizations, showcasing services and benefits.
• Collaborate with local hospitals, physician offices, and assisted living facilities to build robust networks.
• Plan and execute community relations activities to enhance the organization's presence and reputation.
• Ensure effective oral and written communication to facilitate clear and professional interactions.

Job Requirements

• Associate of Arts (AA) degree or equivalent educational background required.
• Minimum of 1 year of experience in patient care liaison roles or similar settings.
• Proven ability to network with healthcare providers and community organizations effectively.
• Knowledge of communication principles, marketing strategies, and community relations practices.
• Strong organizational skills and ability to present ideas to diverse audiences confidently.
• Excellent oral and written communication skills for professional interactions.
• Proficiency in planning and executing social media and outreach campaigns.
