Key Responsibilities & Duties

• Conduct home visits for patient assessments, admissions, and care plan implementations.
• Develop and manage individualized care plans in collaboration with physicians and healthcare teams.
• Perform OASIS assessments and ensure timely and accurate documentation.
• Educate patients and families on health maintenance and disease prevention strategies.
• Maintain compliance with healthcare regulations and organizational standards.
• Report patient progress and concerns to the Clinical Manager regularly.
• Utilize Homecare Homebase software to streamline patient care processes.
• Foster strong relationships with patients to promote trust and effective care delivery.

Job Requirements

• Valid Registered Nurse license and at least one year of nursing experience.
• Associate of Applied Science (AAS) degree in Nursing required.
• Experience in acute care, hospital settings, or home healthcare preferred.
• Proficiency in conducting OASIS assessments and maintaining documentation standards.
• Ability to drive with a personal vehicle for patient visits.
• Familiarity with Homecare Homebase software is advantageous.
• Strong communication skills and ability to work independently.
