Key Responsibilities & Duties
- Conduct home visits for patient assessments, admissions, and care plan implementations.
- Develop and manage individualized care plans in collaboration with physicians and healthcare teams.
- Perform OASIS assessments and ensure timely and accurate documentation.
- Educate patients and families on health maintenance and disease prevention strategies.
- Maintain compliance with healthcare regulations and organizational standards.
- Report patient progress and concerns to the Clinical Manager regularly.
- Utilize Homecare Homebase software to streamline patient care processes.
- Foster strong relationships with patients to promote trust and effective care delivery.
Job Requirements
- Valid Registered Nurse license and at least one year of nursing experience.
- Associate of Applied Science (AAS) degree in Nursing required.
- Experience in acute care, hospital settings, or home healthcare preferred.
- Proficiency in conducting OASIS assessments and maintaining documentation standards.
- Ability to drive with a personal vehicle for patient visits.
- Familiarity with Homecare Homebase software is advantageous.
- Strong communication skills and ability to work independently.
