Overview

  • Lead a dedicated team of hospice nurses, ensuring exceptional patient care and professional development opportunities.
  • Provide strategic guidance and mentorship to nursing staff, fostering a culture of excellence and compassion.
  • Balance administrative responsibilities with fieldwork, including case management and patient care oversight.
  • Collaborate with leadership to implement and uphold hospice care policies and standards.
  • Promote professional development opportunities for nursing staff, encouraging career advancement and skill enhancement.
  • Contribute to the delivery of compassionate and personalized palliative care services.
  • Work in a hybrid environment, combining field visits with administrative duties.
  • Support the hospice nurse manager in achieving operational goals and team success.

Key Responsibilities & Duties

  • Supervise and mentor hospice nursing staff, ensuring adherence to care standards and protocols.
  • Conduct field visits to oversee case management and provide direct patient care as needed.
  • Assist in staff orientation and training, fostering a supportive learning environment.
  • Collaborate with the hospice nurse manager to manage team operations effectively.
  • Develop and implement strategies for staff professional growth and development.
  • Ensure compliance with state regulations and organizational policies in all care activities.
  • Monitor and evaluate staff performance, providing constructive feedback and support.
  • Utilize Microsoft Office tools for documentation and communication purposes.

Job Requirements

  • Bachelor of Science in Nursing (BSN) from an accredited institution.
  • Minimum of two years of nursing experience; four years preferred.
  • New York State RN licensure required; CHPN certification preferred.
  • Proven ability to supervise and mentor nursing staff effectively.
  • Experience in hospice care or related field highly desirable.
  • Proficiency in Microsoft Word, Outlook, and Windows applications.
  • Strong organizational and communication skills, with a focus on patient-centered care.
  • Commitment to obtaining CHPN certification within one year of hire if not already certified.
