Overview
- Manage and supervise case management and social work staff within a healthcare setting, ensuring quality and efficiency.
- Coordinate care assessments for patients, addressing discharge planning and care progression barriers.
- Support multidisciplinary rounds, supervising practices and escalating barriers as needed.
- Participate in complex case meetings to resolve patient care challenges effectively.
- Provide psychosocial services to patients and families, ensuring timely and compassionate support.
- Contribute to departmental development and continuous improvement initiatives.
- Communicate organizational mission and philosophy to staff, patients, and external agencies.
- Participate in professional development and regional/national meetings representing the department.
Key Responsibilities & Duties
- Provide clinical supervision and oversight of case management and social work staff.
- Ensure comprehensive care coordination assessments for patients within 24 hours of admission.
- Support multidisciplinary rounds by modeling best practices and supervising team members.
- Resolve barriers for outlier patients and participate in complex case meetings.
- Deliver patient-centered psychosocial services during normal hours and emergencies.
- Supervise confidential meetings with patients, families, and staff as needed.
- Support departmental budget management and contribute to continuous improvement efforts.
- Maintain knowledge of clinical advancements and healthcare delivery changes.
Job Requirements
- Bachelor of Science in Nursing (BSN) or Master of Social Work (MSW).
- Active RN license or LCSW certification from the State of Illinois.
- Minimum 5 years of experience in medical social work or case management.
- Preferred 7 years of experience in a leadership role within healthcare.
- Strong problem-solving and independent judgment skills.
- Commitment to diversity and equitable treatment within the department.
- Ability to adapt to changes in healthcare delivery systems.
- Experience in multidisciplinary team collaboration and patient care protocol adherence.
