Overview

  • Provide specialized nutritional care to elderly individuals in a dynamic adult day care setting, enhancing their health and well-being.
  • Conduct thorough nutritional assessments to develop tailored care plans that address individual dietary needs and preferences.
  • Collaborate closely with interdisciplinary teams to ensure holistic and effective participant care.
  • Educate participants and families on sustainable healthy eating practices and therapeutic diets.
  • Assist in menu planning and development, incorporating therapeutic and general dietary requirements.
  • Ensure compliance with dietary prescriptions, regulatory standards, and cultural dietary preferences.
  • Participate in quality improvement initiatives to enhance program outcomes and participant satisfaction.
  • Contribute to a supportive and professional environment focused on participant care and development.

Key Responsibilities & Duties

  • Perform initial and ongoing nutritional assessments for program participants to identify specific dietary needs.
  • Collaborate with healthcare professionals during interdisciplinary care plan meetings to ensure comprehensive participant support.
  • Develop and deliver nutrition education materials and training sessions to participants and their families.
  • Plan and modify diets based on medical prescriptions, participant preferences, and cultural considerations.
  • Provide personalized counseling to participants and families to encourage adherence to dietary plans.
  • Support menu development and revisions to meet therapeutic dietary requirements and enhance meal satisfaction.
  • Ensure accurate and timely documentation of nutritional assessments and care plans in compliance with regulations.
  • Assist with meal preparation and distribution tasks, ensuring quality and adherence to dietary guidelines.

Job Requirements

  • Bachelor of Science in Nutrition or a related field is required.
  • Registered Dietitian certification is mandatory for this role.
  • Minimum of 1 year of clinical experience in long-term care or similar settings preferred.
  • Proficiency in assessing nutritional status and developing individualized care plans.
  • Strong documentation skills, adhering to regulatory standards and guidelines.
  • Effective communication and collaboration skills for interdisciplinary teamwork.
  • Knowledge of therapeutic diets, menu planning principles, and cultural dietary considerations.
  • Availability for on-site training and orientation sessions is required.
