Job Description

Overview

  • Deliver personalized nutritional care to participants in an adult day care program, focusing on clinical nutrition and dietary needs.
  • Work on-site in a supportive environment, Monday to Friday, with potential for full-time opportunities.
  • Collaborate with interdisciplinary teams to ensure comprehensive care for elderly individuals with diverse health conditions.
  • Assist in meal planning, preparation, and distribution to meet dietary requirements and preferences.
  • Develop and implement nutrition education materials and training sessions for staff and participants.
  • Ensure compliance with regulatory guidelines and incorporate cultural considerations in dietary planning.
  • Contribute to the organization’s mission by providing dedicated and compassionate nutritional care.

Key Responsibilities & Duties

  • Conduct comprehensive nutritional assessments for participants, revising care plans as necessary.
  • Participate in interdisciplinary care meetings to ensure coordinated and effective care delivery.
  • Evaluate and monitor dietary prescriptions, modifying them in accordance with medical directives.
  • Create and present nutrition education materials and training sessions tailored to staff and participant needs.
  • Plan and adjust therapeutic and general cycle menus to meet diverse dietary requirements.
  • Provide diet counseling and follow-up care to ensure continuity and effectiveness of nutritional services.
  • Support performance improvement initiatives and participate in quality audits within the program.
  • Respect and incorporate cultural and religious dietary preferences in meal planning and service.

Job Requirements

  • Bachelor of Science in Nutrition or a related field is required.
  • Registered Dietitian certification is mandatory, with clinical experience preferred.
  • Proficiency in assessing nutritional status and developing effective care plans.
  • Experience working with elderly populations in care settings such as ADC, SNF, or homecare.
  • Ability to design and revise therapeutic and general cycle menus.
  • Strong communication skills for collaboration and counseling purposes.
  • Commitment to on-site work and participation in required training sessions.
