Overview

  • Provide holistic case management services addressing medical, social, psychological, and spiritual needs of diverse populations.

  • Offer bilingual support in Mandarin or Cantonese to enhance communication and service delivery.

Key Responsibilities & Duties

  • Develop and implement care plans tailored to individual member needs and circumstances.
  • Coordinate with healthcare providers and caregivers to ensure seamless care delivery and support.
  • Conduct assessments to identify and address medical, social, and psychological needs effectively.
  • Provide bilingual support in Mandarin or Cantonese to facilitate clear communication.

Job Requirements

  • Minimum of two years of experience in managed care or related environments.

  • Knowledge of federal and state managed care regulations and guidelines required.
  • Strong written and verbal communication skills; bilingual Mandarin/Cantonese preferred.
