Overview

  • Lead clinical operations for a healthcare organization, ensuring exceptional patient care and regulatory compliance.
  • Collaborate with multidisciplinary teams to develop personalized care plans and optimize service delivery.
  • Supervise clinical staff, providing guidance and ensuring accurate documentation and case management.
  • Participate in quality improvement initiatives to enhance organizational standards and patient outcomes.
  • Act as a resource for inquiries, fostering positive relationships and service excellence.
  • Engage in on-call rotations to support continuous operational effectiveness.
  • Utilize community resources to address patient needs and promote holistic care.
  • Ensure compliance with state and federal regulations, maintaining high standards of care.

Key Responsibilities & Duties

  • Assess and screen patient needs, coordinating intake processes for optimal care delivery.
  • Collaborate with physicians and discharge planners to determine appropriate care levels.
  • Develop and oversee care plans aligned with medical orders and insurance guidelines.
  • Order medical supplies and services, ensuring proper documentation and timely availability.
  • Assign and schedule field staff visits, ensuring timely and effective service delivery.
  • Conduct case conferences to facilitate effective case management and discharge planning.
  • Review documentation for admissions and recertifications to ensure care plan appropriateness.
  • Participate in performance improvement activities to enhance service quality and patient satisfaction.
  • Provide clear direction to support staff, ensuring continuity and consistency in patient care.

Job Requirements

  • Active RN license; BSN preferred.
  • Minimum three years of nursing experience, with two years in home health preferred.
  • Proficiency in OASIS and clinical documentation standards.
  • Strong communication, organizational, and problem-solving skills.
  • Commitment to delivering high-quality service and fostering positive customer relations.
  • Ability to manage multiple responsibilities and prioritize effectively.
  • Knowledge of reimbursement and billing practices.
  • Experience in quality and performance improvement initiatives.
  • Associate of Applied Science degree required; managerial experience preferred.
