Key Responsibilities & Duties
- Assess and screen patient needs, coordinating intake processes for optimal care delivery.
- Collaborate with physicians and discharge planners to determine appropriate care levels.
- Develop and oversee care plans aligned with medical orders and insurance guidelines.
- Order medical supplies and services, ensuring proper documentation and timely availability.
- Assign and schedule field staff visits, ensuring timely and effective service delivery.
- Conduct case conferences to facilitate effective case management and discharge planning.
- Review documentation for admissions and recertifications to ensure care plan appropriateness.
- Participate in performance improvement activities to enhance service quality and patient satisfaction.
- Provide clear direction to support staff, ensuring continuity and consistency in patient care.
Job Requirements
- Active RN license; BSN preferred.
- Minimum three years of nursing experience, with two years in home health preferred.
- Proficiency in OASIS and clinical documentation standards.
- Strong communication, organizational, and problem-solving skills.
- Commitment to delivering high-quality service and fostering positive customer relations.
- Ability to manage multiple responsibilities and prioritize effectively.
- Knowledge of reimbursement and billing practices.
- Experience in quality and performance improvement initiatives.
- Associate of Applied Science degree required; managerial experience preferred.
