Key Responsibilities & Duties

- Perform comprehensive assessments, including OASIS D1, to evaluate patient needs and progress.
- Coordinate with physicians and healthcare professionals to ensure cohesive patient care.
- Document patient care activities and progress using provided digital tools within required timelines.
- Ensure compliance with healthcare regulations and organizational standards in all services.
- Provide culturally sensitive care tailored to individual patient needs and preferences.
- Communicate effectively with patients and their families to foster trust and understanding.
- Report daily activities and patient updates to the Clinical Manager for coordinated care.
Job Requirements
- Associate of Applied Science (AAS) in Nursing or equivalent educational background.
- Valid nursing license with proven experience as a Registered Nurse.
- Minimum of 1 year of experience in acute care, hospital, or home care settings.
- Proficiency in completing OASIS assessments and utilizing healthcare software systems.
- Ability to drive and access to a reliable vehicle for patient visits.
- Strong communication skills and ability to work independently in diverse environments.
