Overview

Key Responsibilities & Duties

  • Conduct home health visits, including patient admissions, care plan updates, and discharges.
  • Perform comprehensive assessments, including OASIS D1, to evaluate patient needs and progress.
  • Coordinate with physicians and healthcare professionals to ensure cohesive patient care.
  • Document patient care activities and progress using provided digital tools within required timelines.
  • Ensure compliance with healthcare regulations and organizational standards in all services.
  • Provide culturally sensitive care tailored to individual patient needs and preferences.
  • Communicate effectively with patients and their families to foster trust and understanding.
  • Report daily activities and patient updates to the Clinical Manager for coordinated care.

Job Requirements

  • Associate of Applied Science (AAS) in Nursing or equivalent educational background.
  • Valid nursing license with proven experience as a Registered Nurse.
  • Minimum of 1 year of experience in acute care, hospital, or home care settings.
  • Proficiency in completing OASIS assessments and utilizing healthcare software systems.
  • Ability to drive and access to a reliable vehicle for patient visits.
  • Strong communication skills and ability to work independently in diverse environments.
