Key Responsibilities & Duties
- Carry a reduced patient caseload, ensuring comprehensive and compassionate hospice care delivery.
- Mentor and supervise nursing staff, fostering professional growth and development.
- Assist with staff orientation and training to ensure readiness and competence.
- Provide leadership and guidance to ensure adherence to hospice program policies and standards.
- Collaborate with leadership on operational and strategic initiatives to enhance care quality.
- Perform fieldwork and case management duties within the Bronx territory.
- Participate in rotating weekend schedules to ensure continuous patient care coverage.
- Utilize Microsoft Office applications for documentation and communication tasks.
Job Requirements
- Associate of Applied Science (AAS) in Nursing; Bachelor of Science in Nursing (BSN) preferred.
- Minimum of two years of nursing experience; four years preferred.
- Active New York State RN licensure; CHPN certification preferred.
- Proficiency in Microsoft Office applications, including Word and Outlook.
- Strong leadership and mentoring skills to supervise hospice nursing staff effectively.
- Ability to perform fieldwork and case management within the Bronx territory.
- Excellent communication and organizational skills to manage team dynamics and patient care.
