Overview

  • Lead and inspire a team of dedicated hospice nurses, ensuring compassionate and high-quality care delivery in alignment with program goals.
  • Balance field-based case management and office responsibilities, fostering professional development and operational excellence.

Key Responsibilities & Duties

  • Provide leadership and mentorship to hospice nursing staff, fostering professional growth and ensuring consistent care delivery.
  • Carry a reduced caseload, focusing on patient care and effective case management.
  • Assist in implementing program policies and achieving organizational goals through collaboration with management.
  • Coordinate orientation and training for new team members, ensuring seamless integration into the team.
  • Perform fieldwork to support staff and ensure adherence to care standards.

Job Requirements

  • Graduation from an accredited School of Nursing; BSN preferred.
  • Minimum of six years of hospice nursing experience, with leadership experience being advantageous.
  • Active New York State RN licensure; CHPN certification preferred or obtainable within one year.
  • Proficiency in administrative tools and software for effective task management.
  • Strong organizational and communication skills to manage diverse responsibilities effectively.
