Overview
- Serve as an Admissions Nurse within a hospice program, facilitating patient transitions with compassion and efficiency.
- Conduct field assessments and admissions, ensuring adherence to hospice care standards and protocols.
- Collaborate with interdisciplinary teams to deliver comprehensive and personalized patient care.
- Manage complex referrals and monitor patient progress within the Access Center.
- Streamline admissions processes under the guidance of the Access Nurse Team Leader.
- Ensure accurate documentation and communication across all involved disciplines.
- Provide exceptional customer service to patients and families during the admission process.
- Participate in weekend shifts to maintain consistent patient care coverage.
Key Responsibilities & Duties
- Evaluate patient eligibility for hospice care, ensuring compliance with program criteria.
- Perform field admissions and assessments, managing caseloads effectively and efficiently.
- Coordinate with Access Coordinators to facilitate timely patient admissions.
- Communicate with interdisciplinary hospice team members to ensure seamless care delivery.
- Verify orders and conduct evaluations prior to patient admission to hospice services.
- Monitor referral lists and manage high-tech and complex cases within the Access Center.
- Uphold professional communication standards and provide excellent customer service.
- Participate in team meetings and contribute to continuous process improvement initiatives.
Job Requirements
- Graduate from an accredited nursing program with an Associate of Applied Science (AAS) degree.
- Hold a valid NY State RN license; BSN preferred but not mandatory.
- Minimum of 2 years of nursing experience; 6 years preferred in acute care, ER, critical care, oncology, or long-term care.
- Proficiency in PC skills and strong written and oral communication abilities.
- Demonstrated ability to work collaboratively in a team-oriented environment.
- Flexibility to travel within designated boroughs using public transportation or rideshare services.
- Availability to work 11 AM to 7 PM shifts and one weekend per month.
- Bilingual Spanish proficiency required for assignments in Manhattan and Bronx.
