Job Description

Overview

  • Provide direct depression care management and clinical services to patients with depression in assigned practices.
  • Collaborate with healthcare team members to deliver high-quality mental health care and services.
  • Utilize independent judgment in delivering care and services, ensuring adherence to professional standards.
  • Engage in psychiatric consultations to enhance patient care and treatment plans.
  • Participate in department and organizational continuous quality improvement projects.

Key Responsibilities & Duties

  • Provide collaborative care services, including direct mental health services and telephonic outreach.
  • Complete patient care documentation promptly, ensuring electronic encounters are closed within 24 hours.
  • Refer patients for long-term counseling and other necessary services, such as substance abuse treatment.
  • Utilize standardized tools for patient assessments and encourage behavioral health workflows.
  • Effectively manage caseloads using reporting tools for self-management.
  • Participate in clinical supervision and contribute to team discussions and strategies.
  • Design and implement clinical pathways and protocols for chronic condition treatment.
  • Engage with community organizations to develop linkages and resources for patient support.

Job Requirements

  • Master’s Degree in Social Work or related field required.
  • NYS licensure as LMSW, LCSW, PhD, or PsyD preferred.
  • Experience working with diverse populations, including HIV, homeless, mentally ill, elderly, and adolescents.
  • Proficiency in Windows-based operating systems and MS Office applications.
  • Familiarity with electronic health records (EHR) technology preferred.
  • Bilingual in Spanish preferred, with additional compensation available.
  • Strong communication and interpersonal skills for effective patient and team interactions.
  • Ability to handle multiple assignments and maintain high standards of care.
