Overview
- Provide direct depression care management and clinical services to patients with depression in assigned practices.
- Collaborate with healthcare team members to deliver high-quality mental health care and services.

Key Responsibilities & Duties
- Provide collaborative care services, including direct mental health services and telephonic outreach.
- Complete patient care documentation promptly, ensuring electronic encounters are closed within 24 hours.
- Refer patients for long-term counseling and other necessary services, such as substance abuse treatment.
- Utilize standardized tools for patient assessments and encourage behavioral health workflows.
- Effectively manage caseloads using reporting tools for self-management.
- Participate in clinical supervision and contribute to team discussions and strategies.
- Design and implement clinical pathways and protocols for chronic condition treatment.
- Engage with community organizations to develop linkages and resources for patient support.
Job Requirements
- Master’s Degree in Social Work or related field required.
- NYS licensure as LMSW, LCSW, PhD, or PsyD preferred.
- Experience working with diverse populations, including HIV, homeless, mentally ill, elderly, and adolescents.
- Proficiency in Windows-based operating systems and MS Office applications.
- Familiarity with electronic health records (EHR) technology preferred.
- Bilingual in Spanish preferred, with additional compensation available.
- Strong communication and interpersonal skills for effective patient and team interactions.
- Ability to handle multiple assignments and maintain high standards of care.
