Overview

Key Responsibilities & Duties
- Develop social work care plans and discharge plans for patients in inpatient facilities.
- Provide direct psychosocial support to patients and families in home care and nursing home settings.
- Collaborate with interdisciplinary teams to address patient and family needs effectively.
- Serve as a liaison between hospice staff and external healthcare providers.

Job Requirements
- Master of Social Work degree from an accredited institution.
