Job Description

Overview

  • Provide psychosocial support and social work services to hospice patients and families in various settings.
  • Develop and implement social work care plans tailored to patient and family needs.
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care.
  • Assist in pre and post-bereavement phases, offering emotional and practical support.
  • Act as a liaison between hospice staff and inpatient/nursing home personnel.
  • Provide field-based services across designated territories, ensuring accessibility to care.
  • Maintain accurate clinical documentation and input data proficiently.
  • Participate in training and supervision to enhance professional skills.

Key Responsibilities & Duties

  • Develop social work care plans and discharge plans for patients in inpatient facilities.
  • Provide direct psychosocial support to patients and families in home care and nursing home settings.
  • Collaborate with interdisciplinary teams to address patient and family needs effectively.
  • Serve as a liaison between hospice staff and external healthcare providers.
  • Conduct fieldwork in assigned territories, ensuring comprehensive patient care coverage.
  • Participate in bereavement support activities for families post-hospice care.
  • Maintain compliance with regulatory and organizational standards in service delivery.
  • Provide supervision hours for LCSW licensure as required.

Job Requirements

  • Master of Social Work degree from an accredited institution.
  • Active LMSW or LCSW licensure in New York State.
  • Minimum of one year of experience in healthcare social work.
  • Proficiency in Microsoft Word and Excel for documentation purposes.
  • Strong organizational, communication, and listening skills.
  • Ability to work effectively within interdisciplinary teams.
  • Experience in hospice or palliative care settings preferred.
