Key Responsibilities & Duties

• Coordinate and implement individualized care plans for home care patients.
• Evaluate patient insurance details and ensure proper documentation and communication.
• Mentor and supervise nursing staff, fostering professional growth and development.
• Ensure adherence to home health regulations and standards in all care activities.
• Conduct field visits to assess patient progress and provide clinical guidance.
• Collaborate with interdisciplinary teams to enhance patient care outcomes.
• Utilize problem-solving skills to address challenges in patient care delivery.
• Maintain accurate records and documentation for all patient interactions.

Job Requirements

• Associate of Applied Science in Nursing; BSN preferred.
• Minimum 6 years of nursing experience, with 3 years in home care required.
• NYS RN license and valid driver’s license are mandatory.
• Oasis Certification and supervisory experience are preferred.
• Strong clinical skills and knowledge of home health regulations.
• Proficiency in critical thinking, multitasking, and organizational abilities.
• Effective communication and documentation skills.
• Ability to work collaboratively within interdisciplinary teams.
