Key Responsibilities & Duties
- Coordinate and implement individualized care plans for home care patients.
- Evaluate patient insurance details and ensure proper documentation and communication.

Job Requirements
- Associate of Applied Science in Nursing; BSN preferred.
- Minimum 6 years of nursing experience, with 3 years in home care required.
- NYS RN license and valid driver’s license are mandatory.
- Oasis Certification and supervisory experience are preferred.
- Strong clinical skills and knowledge of home health regulations.
- Proficiency in critical thinking, multitasking, and organizational abilities.
- Effective communication and documentation skills.
- Ability to work collaboratively within interdisciplinary teams.
