Job Description

Overview

  • Provide specialized pediatric palliative care services, including hospice eligibility assessments and symptom management, to patients in diverse settings.
  • Conduct in-person and telehealth visits, ensuring high-quality care and adherence to best practices in treatment and documentation.
  • Collaborate with interdisciplinary teams to develop and implement comprehensive care plans for pediatric patients.
  • Contribute to the development of training curricula for hospice workers focusing on pediatric care aspects.
  • Engage in quality improvement initiatives to enhance service delivery and patient outcomes.
  • Utilize evidence-based approaches in patient management and team support activities.
  • Coordinate with team physicians and managers to address urgent medical needs of hospice patients.
  • Promote effective communication and goal-setting discussions with patients and families.

Key Responsibilities & Duties

  • Perform face-to-face evaluations for hospice eligibility and manage exigent medical care for pediatric hospice patients.
  • Provide symptom management and treatment recommendations based on thorough history-taking and examination.
  • Support interdisciplinary team members through training and consultation on pediatric hospice care.
  • Develop and implement evidence-based policies and procedures for pediatric palliative care services.
  • Participate in community outreach and long-term care environments as part of service delivery.
  • Utilize Microsoft Word and Excel for documentation and communication purposes.
  • Coordinate resources and personnel in an interdisciplinary team environment effectively.
  • Engage in non-judgmental interactions with patients and families from diverse backgrounds.

Job Requirements

  • Master’s degree in nursing and completion of an approved Nurse Practitioner program.
  • Minimum of 2 years of clinical experience, with 4 years preferred.
  • New York State RN licensure and Certified Nurse Practitioner certification.
  • CPR-BLS certification and valid NYS driver’s license preferred.
  • Hospice & Palliative Care certification encouraged within two years of hire.
  • Proficiency in Microsoft Word and Excel for documentation tasks.
  • Strong communication, teaching, and critical thinking skills.
  • Ability to work flexible hours and independently problem-solve.
