Job Description
Overview
- Provide comprehensive nursing care to clients with severe mental illness within a community-based program.
- Conduct initial assessments, ongoing health monitoring, and medication management as part of a multidisciplinary team.
- Collaborate with psychiatrists and nurse practitioners to ensure effective treatment plans.
- Deliver services in clients' homes, shelters, or other accessible community locations.
- Coordinate care with external medical providers and community organizations.
- Provide health education and promote good health practices among clients and staff.
- Participate in interdisciplinary team meetings and facilitate discussions when required.
- Supervise part-time nursing staff and ensure compliance with standards of care.
- Respond to medical emergencies and provide after-hours emergency coverage on a rotating basis.
Key Responsibilities & Duties
- Carry a caseload, conduct weekly visits, and develop treatment plans tailored to clients' needs.
- Administer prescribed medications, monitor for side effects, and ensure adherence to internal policies.
- Coordinate medication delivery, manage refills, and address insurance authorizations.
- Make referrals, schedule appointments, and accompany clients when necessary.
- Provide advocacy and follow-up care for hospitalized clients to ensure recovery.
- Train staff in health practices and facilitate health education groups for clients.
- Audit program charts monthly to ensure compliance with standards of care.
- Supervise and train part-time nurses, oversee their assignments, and monitor their performance.
- Participate in interdisciplinary team meetings and act as a medical representative.
Job Requirements
- Bachelor's degree in Nursing (BSN) and New York State RN registration.
- Minimum of one year of nursing experience, preferably in behavioral health.
- Strong knowledge of medication management and health monitoring practices.
- Ability to work effectively in a community-based, field-oriented environment.
- Excellent communication and collaboration skills within a multidisciplinary team.
- Proficiency in charting and documentation to meet regulatory standards.
- Flexibility to provide after-hours emergency coverage on a rotating basis.
- Commitment to delivering high-quality care and promoting client well-being.
- Experience in supervising and training nursing staff is preferred.
