Job Description

Overview

  • Serve as a Nurse Case Manager, coordinating care plans for members remotely.
  • Engage with members and families to ensure effective communication and service delivery.
  • Utilize your MLTC experience to enhance member care and satisfaction.
  • Work in a hybrid model, with initial onsite training for 90 days.
  • Collaborate with a dedicated team to deliver person-centered care plans.
  • Document member interactions and care plans thoroughly and accurately.
  • Participate in team meetings to discuss care strategies and member needs.
  • Contribute to the organization’s mission of providing exceptional care services.

Key Responsibilities & Duties

  • Conduct monthly communications with members to review care plans.
  • Coordinate member care with families and vendor agencies effectively.
  • Respond promptly to items requiring attention and follow up as needed.
  • Ensure timely and thorough documentation in member charts.
  • Participate actively in weekly team meetings to discuss member care.
  • Perform assessments and clinical reviews as required.
  • Communicate effectively with members of diverse backgrounds and ages.
  • Adhere to organizational policies and procedures in all duties.

Job Requirements

  • Bachelor of Science (BS) degree required.
  • Minimum of 1 year of experience in MLTC or related field.
  • Proficiency in basic Excel functions such as filtering and sorting.
  • Ability to communicate effectively with members and families.
  • Strong documentation and organizational skills.
  • Knowledge of member assessment and clinical review procedures.
  • Ability to interpret documents and communicate information effectively.
  • No COVID-19 vaccination requirement.
