Key Responsibilities & Duties

Job Requirements
- Active Licensed Practical Nurse (LPN) license is required.
- Associate of Science (AS) degree in Nursing or related field.
- Minimum of 1 year of relevant experience; 3 years preferred.
- Proficiency in medical case management and coordination with healthcare providers.
- Strong documentation skills for maintaining medication records and client progress.
- Ability to provide health education and advocate for client health goals.
