Job Description

Overview

  • Provide medical case management for clients in permanent supportive housing, addressing complex medical and psychiatric needs.
  • Collaborate with healthcare professionals and social services to optimize client outcomes and health goals.
  • Work on-site in a dynamic environment with a dedicated team of professionals.
  • Assist clients with practical needs, including navigating daily challenges and accessing resources.
  • Conduct health education sessions and advocate for client health preferences and goals.
  • Maintain accurate documentation, medication records, and controlled substance logs.
  • Perform emergency first aid and CPR as needed, ensuring immediate care during crises.
  • Participate in treatment team meetings and case conferences to share insights and coordinate care.

Key Responsibilities & Duties

  • Supervise assigned staff and ensure adherence to medical and psychosocial care standards.
  • Link clients to medical treatments and psychiatric rehabilitation services, coordinating with healthcare providers.
  • Conduct health and vitals checks, managing client health needs effectively.
  • Document client progress and maintain medication administration records accurately.
  • Provide direct health education and support to clients, promoting informed health decisions.
  • Assist clients with practical needs, such as understanding bills and arranging services.
  • Ensure adequate medication supply and follow up on medication counts and logs.
  • Collaborate with team members to develop and implement effective client care strategies.

Job Requirements

  • Active Licensed Practical Nurse (LPN) license is required.
  • Associate of Science (AS) degree in Nursing or related field.
  • Minimum of 1 year of relevant experience; 3 years preferred.
  • Proficiency in medical case management and coordination with healthcare providers.
  • Strong documentation skills for maintaining medication records and client progress.
  • Ability to provide health education and advocate for client health goals.
  • Experience in handling emergency situations, including first aid and CPR.
  • Excellent interpersonal skills to work effectively with clients and team members.
