Overview

  • Lead nursing operations for a comprehensive care facility, ensuring exceptional resident-centered services and compliance with health regulations.
  • Oversee clinical care across multiple assisted living programs, fostering quality outcomes and excellence in care.
  • Mentor and develop nursing staff, promoting professional growth and effective teamwork.
  • Collaborate with interdisciplinary teams to create individualized care plans tailored to resident needs.
  • Ensure compliance with state regulations and internal policies to maintain operational standards.
  • Serve as a liaison between residents, families, and healthcare providers, enhancing communication and care coordination.
  • Contribute to the development and refinement of clinical policies and procedures.
  • Manage emergencies and clinical interventions, ensuring resident safety and well-being.

Key Responsibilities & Duties

  • Oversee clinical services within Adult Home, Assisted Living Residence, and specialized care programs.
  • Develop and implement individualized service plans in collaboration with case managers and coordinators.
  • Lead nursing staff, supervising Home Health Aides in medication management and personal care.
  • Conduct resident assessments and reassessments to ensure appropriate care delivery.
  • Maintain accurate clinical documentation and quality assurance reports.
  • Participate in policy development and evaluation to improve care standards.
  • Facilitate communication between residents, families, and interdisciplinary teams.
  • Manage clinical emergencies and interventions effectively.

Job Requirements

  • Current Registered Nurse (RN) license in New York State required.
  • Bachelor’s degree in Nursing (BSN) preferred; equivalent education and experience considered.
  • Advanced training in nursing leadership is advantageous.
  • Minimum 6 years of clinical experience in long-term care or assisted living.
  • At least 4 years in a supervisory or leadership role.
  • Strong knowledge of NYS Assisted Living and Adult Care Facility regulations.
  • Proficiency in Microsoft Office and electronic health record documentation.
  • Exceptional clinical assessment and care planning abilities.
