Nurse Practitioner

Overview

• Provide compassionate, patient-centered primary care services within an assisted living facility, ensuring comprehensive health management for residents.
• Diagnose and manage acute and chronic conditions, tailoring treatments to individual patient needs.
• Prescribe medications responsibly and oversee prescription refills to maintain continuity of care.
• Collaborate with patients, families, and facility staff to create effective care plans.
• Order and interpret diagnostic tests, including laboratory work and imaging, to support clinical decisions.
• Document patient histories and treatment plans accurately in electronic medical records (EMR).
• Work on-site in a dedicated wellness suite, fostering a supportive healthcare environment.
• Collaborate with interdisciplinary healthcare teams to ensure optimal patient outcomes.

• Engage in case discussions with healthcare professionals to enhance care quality.
• Ensure compliance with healthcare regulations and uphold professional standards.

Job Requirements

• Bachelor of Science in Nursing (BSN) degree required.
• Active, unrestricted New York State Nurse Practitioner license.
• Minimum of two years clinical experience; three years preferred.
• DEA certification and PECOS enrollment required.
• Familiarity with eClinicalWorks EMR system preferred.
• Medicare/Medicaid approved provider status necessary.
• Coalition for Affordable Quality Healthcare (CAQH) setup required.
• Basic Cardiac Life Support (BCLS) certification mandatory.
