Key Responsibilities & Duties

  • Assess residents' nutritional needs and create individualized dietary plans to achieve health goals.
  • Monitor residents' progress, document outcomes, and adjust plans as necessary.
  • Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
  • Educate residents and staff on nutrition principles and effective dietary management strategies.
  • Maintain accurate records in compliance with healthcare documentation standards.
  • Participate in quality assurance initiatives to enhance care standards and outcomes.
  • Ensure adherence to State Health Code requirements and other regulatory guidelines.
  • Contribute to team efforts to improve nutritional care practices and resident satisfaction.

Job Requirements

  • Associate of Science (AS) in Food and Nutrition or related field; Bachelor’s degree preferred.
  • Registered Dietitian certification with at least two years of experience in therapeutic nutrition.
  • Experience in long-term care or nursing home settings is highly desirable.
  • Proficiency in healthcare documentation procedures and compliance standards.
  • Strong knowledge of State Health Code requirements and regulatory guidelines.
  • Ability to work on-site during standard business hours, Monday through Friday.
  • Excellent communication and collaboration skills to work effectively within a healthcare team.
