Key Responsibilities & Duties
- Assess residents' nutritional needs and create individualized dietary plans to achieve health goals.
- Monitor residents' progress, document outcomes, and adjust plans as necessary.
- Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
- Educate residents and staff on nutrition principles and effective dietary management strategies.
- Maintain accurate records in compliance with healthcare documentation standards.
- Participate in quality assurance initiatives to enhance care standards and outcomes.
- Ensure adherence to State Health Code requirements and other regulatory guidelines.
- Contribute to team efforts to improve nutritional care practices and resident satisfaction.
Job Requirements
- Associate of Science (AS) in Food and Nutrition or related field; Bachelor’s degree preferred.
- Registered Dietitian certification with at least two years of experience in therapeutic nutrition.
- Experience in long-term care or nursing home settings is highly desirable.
- Proficiency in healthcare documentation procedures and compliance standards.
- Strong knowledge of State Health Code requirements and regulatory guidelines.
- Ability to work on-site during standard business hours, Monday through Friday.
- Excellent communication and collaboration skills to work effectively within a healthcare team.
