Key Responsibilities & Duties

  • Conduct field visits to address specific member needs and provide personalized support.
  • Collaborate with healthcare teams to ensure seamless and coordinated care solutions.
  • Maintain accurate and detailed documentation of member interactions and care plans.
  • Provide exceptional customer service to enhance member satisfaction and trust.

Job Requirements

  • Bachelor of Science in Nursing (BSN) degree is required.
  • Active Registered Nurse (RN) license in the State of New York.
  • Minimum of 1 year of nursing experience, preferably in a related field.
  • Proficiency in UAS tools and healthcare assessment methodologies.
  • Strong interpersonal and communication skills for effective member interaction.
  • Ability to work remotely with occasional travel for field visits.
