Overview
- Lead and mentor a team of hospice nursing staff to ensure high-quality care delivery and compliance with organizational policies.
- Coordinate staff assignments in alignment with established ratios and standards for optimal patient care.
- Provide guidance in pain and symptom management, focusing on patient and family goals of care.
- Monitor and evaluate nursing documentation for adherence to policies and regulatory requirements.
- Participate in interdisciplinary team meetings and collaborate with other departments for effective care planning.
- Engage in community outreach and presentations to promote hospice services.
- Support staff development through training and mentorship opportunities.
- Ensure compliance with state and federal regulations governing hospice care.

Key Responsibilities & Duties
- Supervise and mentor a team of 6–10 full-time and 2–3 fee-for-service hospice staff.
- Coordinate staff schedules and assignments to meet patient care needs.
- Oversee quality assurance processes for nursing documentation and care delivery.
- Facilitate interdisciplinary team meetings and ensure effective communication among team members.
- Provide leadership in pain and symptom management strategies for hospice patients.
- Collaborate with outreach and admissions departments for community engagement activities.
- Participate in training and orientation programs for new staff members.
- Ensure compliance with hospice care standards and regulations.
- Support organizational goals through effective team leadership and strategic planning.
Job Requirements
- Bachelor of Science in Nursing (BSN) from an accredited institution.
- Minimum of five years of hospice nursing experience; supervisory experience preferred.
- Active New York State RN licensure required.
- National Board Certification in Hospice and Palliative Care (CHPN) preferred or obtainable within one year.
- Proficiency in Microsoft Word, Outlook, and Windows applications.
- Strong leadership and mentoring skills to guide hospice nursing staff.
- Ability to work effectively within an interdisciplinary team environment.
