Licensed Practical Nurse (Lpn)

Overview

Key Responsibilities & Duties

• Supervise and coordinate healthcare activities for assigned clients.
• Conduct health assessments and monitor vital signs regularly.
• Document client health status and maintain medication administration records.
• Advocate for client preferences and assist in achieving health goals.
• Collaborate with nursing leadership and team members to enhance care delivery.
• Provide health education tailored to individual client needs.
• Ensure adequate supply and proper handling of medications.
• Participate in treatment planning and interdisciplinary team discussions.
• Perform additional duties as required to support client care initiatives.

Job Requirements

• Active Licensed Practical Nurse (LPN) license required.
• Associate of Applied Science (AAS) in Nursing or related field.
• Minimum of 1 year of experience in a healthcare setting.
• Preferred 2 years of direct patient care experience.
• Proficiency in medical recordkeeping and medication management.
• Ability to provide health education and emergency medical support.
• Strong interpersonal and communication skills for team collaboration.
