Job Description

Overview

  • Provide comprehensive maternal health services to ensure healthy pregnancies and post-partum care for mothers and babies.
  • Conduct telephonic clinical assessments and deliver educational support to pregnant members and new mothers.
  • Assist in scheduling appointments for prenatal, postpartum, and pediatric care, removing barriers to access.
  • Perform medical record audits to ensure quality care and identify areas for improvement.
  • Collaborate with healthcare teams to close quality gaps and enhance maternal and child health outcomes.
  • Educate members on pregnancy guidelines, healthy choices, and early childcare practices.
  • Utilize clinical expertise to provide personalized care and support to members remotely.
  • Contribute to the organization's maternal health initiatives and quality improvement programs.

Key Responsibilities & Duties

  • Schedule and coordinate prenatal, postpartum, and pediatric care appointments for members.
  • Conduct telephonic post-partum assessments to ensure recovery and identify care needs.
  • Provide educational resources on pregnancy, post-partum care, and childcare.
  • Perform targeted medical record reviews to ensure adherence to clinical guidelines.
  • Summarize audit findings and collaborate with teams to address identified issues.
  • Assist members in overcoming barriers to accessing necessary healthcare services.
  • Enhance maternal and child health outcomes through proactive outreach and education.
  • Maintain accurate documentation and reporting of clinical assessments and audits.

Job Requirements

  • Bachelor of Science in Nursing (BSN) and New York State Nurse Midwife license.
  • Minimum of 3 years of experience in maternal health or managed care environment.
  • Advanced clinical assessment skills and ability to interpret medical records.
  • Proficiency in computer applications, including word processing and database programs.
  • Excellent organizational, written, and verbal communication skills.
  • Strong interpersonal skills and ability to work collaboratively with healthcare teams.
  • Experience in quality improvement initiatives and maternal health education preferred.
