Job Description

Overview

  • Provide comprehensive primary care services to a diverse patient population in a collaborative healthcare setting.
  • Specialize in HIV care and transgender health, ensuring patient-centered and respectful medical practices.
  • Deliver care within scheduled appointments, ensuring timely follow-up on patient needs and referrals.
  • Work within a multidisciplinary team to provide coordinated care and maintain high-quality standards.
  • Participate in quality improvement initiatives and professional development opportunities.
  • Flexible scheduling options, including daytime, evening, and optional Saturday shifts.
  • Utilize electronic health records (EPIC) for accurate and prompt documentation of patient care.

Key Responsibilities & Duties

  • Conduct patient screenings, examinations, diagnoses, and treatments across various age groups.
  • Provide evidence-based care that is respectful, compassionate, and unbiased.
  • Collaborate with healthcare professionals to ensure coordinated patient care.
  • Make appropriate referrals to internal and external specialists as needed.
  • Participate in case conferences and quality improvement initiatives.
  • Engage in continuing medical education to stay updated on best practices.
  • Document all patient interactions and care promptly using EPIC.
  • Deliver care within scheduled appointment times, ensuring efficiency and patient satisfaction.

Job Requirements

  • Doctor of Medicine (MD) with New York State Certification/Licensure.
  • Current NYS Registration and DEA Controlled Substances Registration Certificate.
  • Board certified or eligible in Family Medicine, Internal Medicine, or Infectious Disease.
  • Minimum of 2 years of primary care experience; 3 years preferred.
  • Experience in community-based primary care settings is highly desirable.
  • Proficiency in bilingual communication (Spanish/English) is a plus.
  • Strong collaboration skills within a team-based care model.
