Job Description
Overview
- Provide clinical nutrition services for participants in an Adult Day Program.
- Work part-time, Monday to Friday, 9 AM to 2 PM.
- Support older adults with cognitive or physical limitations.
- Ensure compliance with dietary prescriptions and physician orders.
- Collaborate with interdisciplinary teams for care planning.
- Potential for full-time conversion as the program expands.
- Contribute to the development of nutrition education materials.
- Participate in in-service training and audits as required.
Key Responsibilities & Duties
- Conduct initial and quarterly nutritional assessments for clients.
- Plan and modify diets based on physician prescriptions.
- Monitor and adjust diet prescriptions as needed.
- Provide diet counseling and education for clients.
- Assist in the development of cycle menus for therapeutic diets.
- Participate in interdisciplinary care plan meetings.
- Support meal preparation and distribution activities.
- Respect cultural and religious dietary preferences of participants.
Job Requirements
- Registered Dietitian or Nutritionist certification required.
- Clinical experience in long-term care preferred.
- Ability to assess nutritional status and document care plans effectively.
- Experience working with elderly populations in similar settings.
- Strong communication skills for interdisciplinary collaboration.
- Knowledge of regulatory guidelines for nutritional documentation.
- High School Diploma or GED is required.
- On-site training provided; familiarity with EMR systems is beneficial.
