Job Description

Overview

  • Deliver compassionate hospice care, ensuring patient dignity and comfort during end-of-life transitions.
  • Conduct comprehensive assessments to create personalized care plans addressing diverse patient needs.
  • Collaborate with interdisciplinary teams to provide holistic care and optimize outcomes.
  • Educate patients and families on hospice services, fostering understanding and support.
  • Supervise and mentor healthcare staff to uphold high-quality care standards.
  • Implement physician orders and adapt care plans to evolving patient conditions.
  • Participate in quality improvement initiatives to enhance care standards and satisfaction.
  • Maintain accurate documentation to ensure compliance and effective communication.

Key Responsibilities & Duties

  • Conduct detailed assessments of patients' physical, emotional, and social conditions.
  • Develop and implement individualized nursing care plans tailored to patient needs.
  • Supervise and evaluate the performance of home health aides and other care staff.
  • Coordinate with physicians to update and modify care plans as necessary.
  • Provide hands-on care and emotional support to patients and their families.
  • Educate families on hospice care processes and available resources.
  • Ensure compliance with regulatory standards and organizational policies.
  • Participate in interdisciplinary team meetings to discuss patient progress and care strategies.

Job Requirements

  • Associate of Science (AS) in Nursing or equivalent education required.
  • Active Registered Nurse (RN) license with a minimum of 1 year of clinical experience.
  • Preferred experience in hospice, home care, or acute care settings.
  • Exceptional communication, teaching, and interpersonal skills.
  • Ability to work effectively within an interdisciplinary team environment.
  • Proficiency in clinical data input and accurate documentation.
