Job Description

Overview

  • Perform comprehensive assessments using the Uniform Assessment System (UAS-NY) to evaluate members' clinical, psychological, and social needs.
  • Collaborate with interdisciplinary teams to design personalized care plans aligned with members' preferences and health requirements.
  • Conduct hybrid assessments, including telephonic and in-home visits, ensuring adherence to regulatory standards and guidelines.
  • Educate members and families on program benefits, eligibility criteria, and available care options in a clear and accessible manner.
  • Ensure accurate documentation and timely submission of assessment results to relevant teams for review and action.
  • Perform initial enrollment assessments and periodic reassessments, focusing on safety, eligibility, and care planning.
  • Support members in accessing services, including CDPAS enrollment, and coordinate with healthcare professionals for optimal care delivery.
  • Participate in ongoing training sessions to stay updated on policies, procedures, and best practices in assessment and care planning.

Key Responsibilities & Duties

  • Conduct UAS-NY assessments to determine member needs and eligibility for services, ensuring compliance with established guidelines.
  • Develop and communicate individualized care plans to members, families, and interdisciplinary teams effectively.
  • Ensure timely completion and accurate documentation of all assessments within mandated timeframes.
  • Coordinate with interdisciplinary teams to address urgent care needs and safety concerns promptly and effectively.
  • Educate members about program benefits, responsibilities, and care options during assessment processes.
  • Conduct follow-up assessments to ensure members receive appropriate care and services tailored to their needs.
  • Maintain compliance with Department of Health policies and regulations during all assessment activities.
  • Engage in professional development activities to enhance assessment skills and knowledge for improved service delivery.

Job Requirements

  • Associate Degree in Nursing (AAS) required; Bachelor's Degree in Nursing preferred for advanced care planning expertise.
  • Minimum of two years of experience in community health, managed care, or related fields essential for effective assessments.
  • Proficiency in Microsoft Word, Excel, and UAS-NY systems for documentation and reporting purposes.
  • Bilingual skills in Bangla, Hindi, Urdu, Mandarin, Cantonese, or Spanish are highly desirable for diverse member engagement.
  • Valid driver's license and willingness to travel within assigned territories for in-home assessments.
  • Comfortable conducting both telephonic and in-home assessments in a hybrid work environment.
  • Strong communication and interpersonal skills to interact effectively with diverse populations.
  • Ability to manage multiple cases and meet productivity targets efficiently in a dynamic setting.
