Job Requirements
- Bachelor's degree in Nursing (BSN) required; Master's degree in a related field preferred.
- Active RN, LCSW, or LMSW licensure required.
- Minimum of 3 years of acute care hospital case management experience required.
- At least 2 years of leadership experience in case management within acute care settings.
- Proven ability to achieve performance and quality objectives in healthcare environments.
- Strong analytical skills and experience with performance improvement methodologies.
- Excellent communication, leadership, and motivational abilities.
