Key Responsibilities & Duties

  • Provide direct patient care under the supervision of a registered nurse, adhering to NYS Nurse Practice Act guidelines.
  • Coordinate all aspects of patient care, ensuring adherence to professional standards and practices.
  • Document services rendered accurately and timely in the patient’s plan of care.
  • Assist in patient and family counseling and education, promoting understanding and comfort.
  • Provide pain and symptom management according to established care plans.
  • Maintain confidentiality and adhere to organizational policies and procedures.
  • Participate in interdisciplinary assessments and quality improvement initiatives.
  • Support team goals and contribute to a positive work environment.

Job Requirements

  • Graduate of an accredited practical nursing program with a valid NYS Practical Nurse license.
  • Minimum of two years of clinical nursing experience, preferably in Med-Surg settings.
  • Proficiency in electronic medical records documentation and critical thinking skills.
  • Commitment to the hospice philosophy of care and professional standards.
  • Ability to work weekends and holidays as needed, with flexible scheduling.
  • Strong teamwork and communication skills to collaborate effectively with diverse populations.
