Key Responsibilities & Duties
- Provide direct patient care under the supervision of a registered nurse, adhering to NYS Nurse Practice Act guidelines.
- Coordinate all aspects of patient care, ensuring adherence to professional standards and practices.
- Document services rendered accurately and timely in the patient’s plan of care.
- Assist in patient and family counseling and education, promoting understanding and comfort.
- Provide pain and symptom management according to established care plans.
- Maintain confidentiality and adhere to organizational policies and procedures.
- Participate in interdisciplinary assessments and quality improvement initiatives.
- Support team goals and contribute to a positive work environment.
Job Requirements
- Graduate of an accredited practical nursing program with a valid NYS Practical Nurse license.
- Minimum of two years of clinical nursing experience, preferably in Med-Surg settings.
- Proficiency in electronic medical records documentation and critical thinking skills.
- Commitment to the hospice philosophy of care and professional standards.
- Ability to work weekends and holidays as needed, with flexible scheduling.
- Strong teamwork and communication skills to collaborate effectively with diverse populations.
